2009-2010 School Year New Student Enrollment Steps

 1. Complete the following forms:
  • New Student Enrollment Form
  • Guardianship Status Form
  • Records Request Form
  • Health Form
  • Internet Consent Form
  • Course Request Worksheet 7th grade / 8th grade / 9th grade
These forms are available in the Timberline Counseling Office or on our website by scrolling to the bottom of this page.

2. Collect the following documentation:
  • Proofs of Residency - List of acceptable residency proof.
  • Proof of Immunizations - Student must be up to date on all required immunizations to begin attending school.
  • Birth Certificate - Birth Certificate must be an original certified copy. Heritage/souvenir birth certificates are not acceptable.
  • IEP- If a student has been serviced in Special Education, you must provide a copy of the current IEP.
  • Custody Documentation - If parents are divorced or there are any issues of guardianship, a copy of the custodial rights, signed by a judge, must be provided at time of enrollment.  
  • Withdrawal Form - Student must withdraw from their prior school before enrolling.
*Please note that some situations require additional documentation/forms that may be required before registration may be completed.

3. Call the Timberline Counseling Office to schedule an appointment for enrollment at 801-763-7046.

Living With Another Family Form -
Required if the student's family resides in the home of another family.

Home School Exemption -
Required if the student will be on release time.

Out of Area Transfer Request -
Required to request approval for a student to attend an out of boundary school.
